چکیده

Background. Decreased spinal excitability contributes to the immobilizing effects of halogenated ethers during general anaesthesia. Recurrent spinal responses such as F-waves reflect spinal excitability and are suppressed by volatile anaesthetics. To evaluate whether F-waves are suitable for monitoring immobility, the concentration-dependent effects of sevoflurane on F-waves were compared with effects on the Bispectral Index (BIS ). The predictive power of all parameters for movement responses to noxious stimuli was tested. In addition, the effect of the noxious stimulus itself on F-waves was investigated.

NEUROSCIENCES AND NEUROANAESTHESIA Sevoflurane pre- and post-conditioning protect the brain via the mitochondrial KATP channel

Methods. Adult Wistar male rats were subjected to focal cerebral ischaemia for 1 h followed by 24 h or 7 days of reperfusion. Preconditioning consisted of 15 min exposure to sevoflurane at 1 minimum alveolar concentration (2.6%) 72 h before ischaemia. Post-conditioning was performed by exposure to sevoflurane immediately at the onset of reperfusion or by a delayed exposure 5 min after the onset...

NEUROSCIENCES AND NEUROANAESTHESIA Do we need inhaled anaesthetics to blunt arousal, haemodynamic responses to intubation after i.v. induction with propofol, remifentanil, rocuronium?

Methods. One hundred ASA I and II patients were randomized to receive 1 MAC of desflurane or sevoflurane during manual ventilation or not. Anaesthesia was induced with an effect-sitecontrolled infusion of remifentanil at 2 ng ml 1 for 3 min. Patients then received propofol to induce loss of consciousness (LOC). Rocuronium (0.6 mg kg ) was given at LOC and the trachea was intubated after 90 s of...

NEUROSCIENCES AND NEUROANAESTHESIA Changes in axonal excitability of primary sensory afferents with general anaesthesia in humans

Key points † Intraoperative neuromonitoring can be affected by specific general anaesthetics. † Changes in axonal excitability of primary sensory afferents with propofol and sevoflurane were investigated using threshold tracking technique. † Both anaesthetics similarly affected nerve excitability to a small but significant extent. † Possible mechanisms include direct anaesthetic effects on neur...

NEUROSCIENCES AND NEUROANAESTHESIA Surveyon the adequacyof depth of anaesthesia with bispectral index and isolated forearm technique in elective Caesarean section under general anaesthesia with sevoflurane

Editor’s key points † Bispectral index (BIS) monitoring is commonly used to monitor depth of anaesthesia. † The authors studied parturients in whom standard techniques were used for general anaesthesia for Caesarean section. † The isolated forearm test was used to study the ability of the BIS to detect consciousness. † BIS values below 60 were commonly associated with IFT responses, particularl...
